What is a Home Care Package?
A Home Care Package (HCP) offers coordinated care and services to support individuals with basic to more complex aged-related care needs, enabling them to remain in their homes for as long as possible.
Home Care Packages are allocated according to your assessed level of care needs.
There are four levels available from level one, if you require basic support, through to level four if you have more complex care needs.
Care plans include a range of services tailored to your assessed needs and goals.

How to access a Home Care Package
You may be eligible for a Home Care Package if:
- you need help with some everyday tasks
- you are 65 years or older (50 years or older for Aboriginal or Torres Strait Islander people)
- you are on a low income, homeless or at risk of being homeless,  and aged 50 years or older (45 years or older for Aboriginal and Torres Strait Islander people)
The first step is to apply for an aged care assessment through the Australian Government’s My Aged Care website or by calling My Aged Care on 1800 200 422.

Our clinical care
Royal Freemasons Home Care Packages are overseen by a team of specialist case managers, who are also registered nurses. They provide clinical oversight of care planning and regularly review your care.
We also have a team of skilled and experienced general case managers, who coordinate your care according to your changing needs so you can continue to live safely and independently at home for as long as possible.
