															Support at Home (SAH) offers coordinated care and services to support your basic to more complex aged-related care needs, enabling you to remain at home for as long as possible.
The Support at Home program is allocated according to your assessed level of care needs.
There are eight levels available from level one, if you require basic support, through to level eight if you have more complex care needs.
Care plans include a range of services tailored to your assessed needs and goals.
The Royal Freemasons Support at Home program is overseen by a team of specialist case managers, who are also registered nurses. They provide clinical oversight of care planning and regularly review your care.
We also have a team of skilled and experienced general case managers, who coordinate your care according to your changing needs so you can continue to live safely and independently at home for as long as possible.

While at first I didnโt think personal care for Herbert was necessary for more than twice a week, Iโve now realised itโs a great help to me both physically and mentally.
