What is a Home Care Package?

A Home Care Package (HCP) offers coordinated care and services to support individuals with basic to more complex aged-related care needs, enabling them to remain in their homes for as long as possible.

Home Care Packages are allocated according to your assessed level of care needs.

There are four levels available from level one, if you require basic support, through to level four if you have more complex care needs.

Care plans include a range of services tailored to your assessed needs and goals.

Our clinical care

Royal Freemasons Home Care Packages are overseen by a team of specialist case managers, who are also registered nurses. They provide clinical oversight of care planning and regularly review your care.

We also have a team of skilled and experienced general case managers, who coordinate your care according to your changing needs so you can continue to live safely and independently at home for as long as possible.

How to access a Home Care Package

You may be eligible for a Home Care Package if:

The first step is to apply for an aged care assessment through the Australian Government’s My Aged Care website or by calling My Aged Care on 1800 200 422.

FAQs

You may be eligible for a Home Care Package if you:

  • need help with some everyday tasks
  • are 65 years or older (50 years or older for Aboriginal or Torres Strait Islander people)
  • are on a low income, homeless or at risk of being homeless, and aged 50 years or older (45 years or older for Aboriginal and Torres Strait Islander people)

The first step is to apply for an aged care assessment by visiting the Australian Government’s My Aged Care website: myagedcare.gov.au/am-i-eligible or calling My Aged Care on 1800 200 422.

The Home Care Package is designed to support you with expenses relating to care and support as you age. It is not meant as a form of income. This means your Home Care Package cannot be used to pay for everyday living expenses, such as:

 

  • groceries
  • rent of mortgage payments
  • utilities
  • rates
  • general home services not related to supporting care needs
  • general household furniture and appliances not specifically designed to support aging-related care needs
  • modifications or capital items unrelated to your care needs
  • extensive gardening services
  • payment of fees or charges for care or services funded or jointly funded by the Australian Government
  • payment for services and items covered by the Medicare Benefits Schedule (MBS) or the Pharmaceutical Benefits Scheme (PBS).
