A PAPER ON THE ‘FOR YOU FOR LIFE’ TAPESTRY PROGRAM HAS JUST BEEN ACCEPTED FOR PUBLICATION IN GERIATRIC NURSING, AN ESTEEMED AMERICAN JOURNAL, AND WILL PROVIDE INTERNATIONAL EXPOSURE FOR THIS VERY VALUABLE WORK.
For You For Life is an enhanced care program designed by Royal Freemasons and the National Ageing Research Institute (NARI). It is one of the services that we have designed to track, integrate and maintain the consistency of all the medical, functional and emotional requirements and treatments of an individual, from the moment they enter our care and through all the stages of their journey with us. A central feature is the Transition Map.
It gives Royal Freemasons staff a streamlined and well considered person-centred model of care that encourages transparency, collaboration, and coordination of approach to the care of each resident.
The program was successfully piloted at Coppin Lodge and Colbran Lodge at Coppin Centre and has since rolled out to our facilities through staff education.
Recent evaluation of the program by NARI and Royal Freemasons Senior Geriatrician, Associate Professor Dr Sam Scherer, proved to be extremely positive.
Most importantly, the residents’ outcomes were very affirming. Our application to further develop, implement and evaluate the For You For Life program has been shortlisted for funding by the Department of Health, as part of the Dementia and Aged Care Services Research and Innovation Funding Round.
These grants support projects that tackle existing and emerging challenges in dementia. We plan to further expand the For You For Life Tapestry program to the new Royal Freemasons regional facilities to promote its valuable benefits to our residents and the integrated ways we care for them.
