The new Consumer Directed Care model is about working in partnership with our customers.
Changes to the Home Care model are improving the organisation’s ability to enhance the health and wellbeing of those living in their own home by broadening the capabilities and expertise of our team, extending contact hours and using supporting technology.
One new program already implemented as a trial is tele-monitoring equipment. This enables our customers to monitor some of their health conditions, such as blood pressure and blood glucose, with the support of qualified Royal Freemasons staff.
In response to customer feedback, office hours have also been extended and are now 7am-6.30pm.
