Independent and safe at home

Support at home is a government-funded program, which enables you to remain at home safely and independently.

At Royal Freemasons, we can provide you with personalised Support at Home (SAH) services and supports to maintain your independence and quality of life.Β 

Through the SAH program, our experienced care managers can support you to access a range of coordinated services to meet your individual needs, based on your assessment decision.

Accessing Support at Home

Contact us

Accessing aged care can be daunting. Contact our expert team and we will guide you through the process.

Register with My Aged Care

Register with My Aged Care to find out if you are eligible for Support at Home. Telephone 1800 200 422 or visit myagedcare.gov.au/ .

Assessment

You will be referred for an Aged Care Assessment to determine the level of care and funding you require.

Approval and waitlist

You will be notified if you are approved for a package and your name will be added to the national waiting list.

Financial assessment

Depending on your situation and the services you have been approved for the Government may require you to contribute to the cost of your care.

Do I contribute to the cost of my services?

Services Australia (or the Department of Veterans’ Affairs, if applicable) determine what each participant is required to pay.

They will conduct an income and asset assessment to determine your contribution rate. If your financial details are not known (i.e. you are not a pensioner), you will need to complete an income and assets assessment request. You will have 28 days from when Services Australia contact you to provide these details. If you do not provide income and assets information, your contribution will be set at the maximum rate.
