A Guide To The Admission Process

The decision to move to Coppin Centre, a residential low care facility, is a major one in the life of each resident. While our experience has shown that it is a decision few people regret, it is important that you are fully aware of all aspects of life at the Coppin Centre before making your decision. To assist you in navigating and applying for a place, we have produced this easy to follow guide, which should be read in conjunction with the other material contained in this information pack.

Tours

We believe it is very important that you visit the Coppin Centre before you apply. Tours are conducted during working hours. To may make an appointment for a tour, contact the Director of Nursing, whose details are listed over leaf.

The Admission Process

We do not keep a long waiting list for admissions, so you should only apply if you are seeking admission within the next three months.

Stage 1

To apply for admission you will need to complete and submit:

  1. An Application for Admission form, included within this pack
  2. A Personal Details of Applicant form, included within this pack
  3. A Consent to Exchange Information form required by privacy legislation, included within this pack
  4. A copy of a current Aged Care Assessment Service (ACAS) assessment. Your doctor can arrange this at your request
  5. A statement from Centrelink or Department of Veterans Affairs regarding your assets. This can be arranged when you get your ACAS assessment. Alternatively, you may prefer to simply provide us with a letter advising that you will pay the maximum accommodation charge. If you do not have a statement of your assets you can still apply however you will need to pay the maximum accommodation charge on an interim basis and then we will adjust the charge, if necessary, when you provide us with the statement. In the case of applications from couples, separate application forms are required from each partner.

Stage 2

Your application will be assessed by our staff. As part of this process, we may request further information or ask to meet with you to more accurately determine your care needs.

You will then be advised in writing regarding the outcome of your application. In some cases applicants are advised that we are not able to offer them admission or that we are uncertain of our ability to offer them admission and they will be encouraged to also pursue other accommodation options. We may also seek permission to refer the application to another Royal Freemasons facility that may be more suitable.

Stage 3

If your application is accepted, you will be placed on the waiting list and your letter will state advice (if applicable) regarding the Accommodation Bond you will be required to pay.

Stage 4

When a suitable room becomes available, we will contact you and provide you with two copies of the Resident Agreement. If you are required to pay an accommodation bond you will need to sign the Agreement prior to admission. A Direct Debit Request form will also be supplied. You will only need to complete this form upon admission if you want to pay your monthly account by direct debit.

Stage 5

You will receive an account for your weekly fee during the first month of your occupancy. If you are required to pay an accommodation bond, you have up to six months to pay, but interest will apply from the date of admission and if not paid within the first month interest will also apply from date of admission.

Respite Care

In addition to permanent admission we also provide Respite Care. This provides short-term temporary admission during periods of recovery from illness, to cover the absence of a carer or to allow you to trial life at the Coppin Centre.
