Continued Care

Royal Freemasons commitment to a continuum of care means Community Care clients receive the necessary ongoing care ongoing without having to move locations.

As needs change, Royal Freemasons are able to facilitate the client's smooth transition through the different care packages, for examples moving from a CACP to an EACH package. We are also able organise priority access to either our low care residential accommodation at Coppin Centre or one of our high care Lodges should it be required.

If you wish to speak to someone regarding any aspect of your Community Care package then you should contact your Care Manager. They will provide you with ongoing support, consultation and be involved in decision-making about all aspects of your care.

This could including identifying appropriate levels of care, monitoring your ongoing needs and the delivery of your services, providing instructions for the staff that will assist you and ensuring all elements of your package are administered efficiently.

To ensure that any change in circumstances or requirements is adequately addressed, formal care plan reviews occur annually. During the annual visit, your Care Manager will discuss any changes in services that you may need and develop a new care plan, which will then be sent out to you.

In addition to the annual reassessment and care plan review, your Care Manager will contact you every two months to discuss your care plan. This may be a phone call or a visit from your Care Manager. During the two-monthly review, your Care Manager will discuss any health issues you may be experiencing, the suitability of the services you are receiving, and whether any actions need to be taken to improve your individual circumstances.

These reviews ensure your care plan is reviewed formally at least six times each year. If an issue arises requiring prompt resolution, you are able to contact your Care Manager directly by phone.

All aspects of the packages are delivered in accordance with the community care standards, as specified by the Commonwealth Government who fund, regulate and monitor the program. This ensures that clients receive only the highest quality care and are given the required rights and responsibilities.

Respite Care

Royal Freemasons also offers a respite service to offer support to partners, family members or carers of clients who receive our Community Care packages. Respite care can be arranged to give carers the opportunity to take a break from what can be demanding and challenging role.

Day Therapy

Clients who receive our Community Care packages are welcome to visit the Royal Freemasons Day Therapy, located at our Coppin Centre at the organisation's Moubray Street home, just off St Kilda Road.

The focus of Day Therapy is on restoring independence and improving the general health and well being of clients following illness or injury by developing a tailored program following an assessment of their needs

The centre offers a wide range of therapy and rehabilitation services including, physiotherapy, occupational therapy, podiatry, audiology, speech pathology as well as other specialist clinics, classes and treatments.
