Taking our care, compassion and support out into the community and into your home.

As we get older, the familiar and comforting surroundings of a home environment and a friendly local community become more and more important. In your own home you feel safe, secure and content. But for many elderly people, the daily challenges of looking after themselves and their property can become too much. Royal Freemasons' Community Care packages help clients to live a dignified and rewarding life by providing the quality care and professional services they need to enable them to remain at home.

Operating from our Outreach offices located across metropolitan Melbourne, Royal Freemasons offer three individually planned and coordinated packages of care catering for the varying needs of those clients who would be eligible for low, high and dementia residential care, but choose to live in their own home.

As the cost associated with delivery of Community Care packages is subsidised by the Commonwealth Government, you must be assessed by an Aged Care Assessment Service (ACAS), as requiring the necessary level care. The ACAS assessments are free of charge and further information is available from doctors, hospitals and community centres, the Aged and Community Care Information Line on 1800 500 853, or Commonwealth Respite and Carelink Centres on 1800 052 222.

The Aged Care Assessment Service will determine which type of Community Care package best suits your needs. Currently these packages comprise the following.

Community Aged Care Packages (CACP)

Community Aged Care Packages deliver a range of care and other domestic services to clients who would be eligible for low care. Services and care offered as part of a CACP Package may include the following:

  • Supervision and assistance with showering, bathing and personal hygiene, dressing, undressing, mobility and monitoring of medications
  • Assistance with the provision of meals
  • Home care for domestic tasks such as cleaning, laundry and shopping
  • Assistance with transportation to appointments and social activities
  • Ongoing coordination and management of care
  • 24-hour emergency contact

Extended Aged Care at Home Packages (EACH)

Extended Aged Care at Home Packages provide individually planned and coordinated packages of care for clients with high care needs. An EACH package may include the above CACP care and services and also where required:

  • Technical nursing care
  • Allied health care
  • Continence management aids
  • Assistance with oxygen and/or enteral feeding

Extended Aged Care at Home Dementia Packages
(EACH Dementia)

Extended Aged Care at Home Dementia Packages, are designed for clients experiencing difficulties due to the behavioural and psychological symptoms associated with dementia. Packages may include all of the above care and services as well as other services and strategies to meet the specific needs of care recipients.

The Aged Care Assessment Service will place your details on a central database so providers in your local area are made aware of your requirements. Should you decide that you would prefer your package to be delivered by Royal Freemasons you should advise the ACAS and contact us directly by using the Register Your Interest form or by telephoning the our office that serves your local area.
